Job summary
Brookhaven Hospice is seeking a Per Diem Licensed Nursing Assistant (LNA) to join our compassionate team in Windham, NH. You will provide hands-on care that brings comfort and dignity to patients at the end of life, working closely with our nursing staff to ensure needs are met with respect and professionalism.

Qualifications
- High School Diploma or GED required.
- Current unencumbered LNA license in the state.
- Minimum one year LNA experience in acute care or LTC; hospice exp a plus.
- Current driver's license, vehicle insurance, and reliable transportation.
Responsibilities
- Provide Personalized Care following the plan of care for patient comfort and dignity.
- Assist with personal care needs including bathing and hygiene.
- Deliver exceptional customer service to patients and families.
- Support families with guidance and knowledge during end-of-life care.
- Maintain accurate documentation in home and agency records.
