Mercy Home Care is seeking a Registered Nurse (RN) to provide day-to-day patient care in patients’ homes within the Saint Louis area. You will perform post-hospitalization assessments, assist with admissions and discharges, and educate patients on care plans while ensuring safety and quality standards.

The role requires current Missouri RN licensure, home health or acute care experience, and proficiency with IV therapy.

Qualifications

• Active Missouri RN license.
• Clinical nursing experience in home health or acute care.
• Ability to perform post-hospitalization assessments and discharge planning.
• Experience with IV therapy and patient teaching.

Responsabilités

• Provide day-to-day patient care in the home setting.
• Conduct post-hospitalization assessments and recertifications.
• Coordinate admissions and discharges for Mercy Home Care patients.
• Document visits timely in Epic and ensure safety.
