Job summary

Amedisys Home Health is seeking a Registered Nurse to perform adult home health visits in Lake Charles, LA. This per diem role offers flexible daytime hours and the chance to impact patients’ lives at home.

Requires at least 6 months of home health experience, an active RN license in the relevant state, CPR certification, a valid driver’s license, reliable transportation, and a willingness to collaborate with care teams to deliver high-quality home-based care.

Qualifications

• One year of clinical experience as an RN; if under 1 year, approval by VP clinical.
• Current RN license for the state(s) assigned.
• Minimum six months home health experience.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.
• Ability to assess patient status and navigate computer/HR systems.
• RN Intern program if less than 6 months of RN clinical experience.

Responsibilities

• Coordinates and oversees all aspects of patients' home care.
• Collaborates with patient care teams to develop and implement care plans.
• Educates families on patient condition, needs and care.
• Performs head to toe physical assessments.
• Checks vital signs and manages medications.
• Gives injections, IV therapy or tube feedings.
• Administers and maintains catheters.
• Completes documentation timely and accurately.
• Supervises LPNs and HHAs.
• Regularly communicates patient progress to the clinical manager, physician and care team.
• Other duties as assigned.
