Job summary

Lehigh Valley Health Network (LVHN) is seeking a Registered Nurse for Home Care – per diem II to coordinate and implement individualized home health plans. The role involves close collaboration with physicians, patients, and families to ensure continuity of care and effective symptom management.

Responsibilities include comprehensive assessments, following care plans, administering skilled interventions, and educating patients and caregivers on end-of-life care, palliative options, and discharge

Qualifications

• Associate Degree in nursing or a Specialized Diploma in nursing.
• 1 year of direct care experience in a healthcare setting.
• Knowledge of patient education techniques and principles.
• Knowledge of Home Health Conditions of Participation.
• American Heart Association Basic Life Support - State of Pennsylvania.
• Upon Hire RN - Licensed Registered Nurse_PA - State of Pennsylvania.
• Upon Hire Driver's License_PA - State of Pennsylvania.

Responsibilities

• Performs comprehensive initial and ongoing assessments.
• Follows an established plan of care under the RN Care Manager and physician orders.
• Performs skilled treatments and interventions.
• Determines home health level of care and skilled needs.
• Provides education to patients, families, and clinical staff on end-of-life care, palliative care, and discharge planning.
• Promotes patient and caregiver autonomy and evaluates teaching effectiveness.
