Job summary

Lehigh Valley Health Network is seeking an on-call Registered Nurse for Home Care to provide daytime and on-call nursing support in patients' homes. You will perform triage, direct care, education, and coordination with the care team to meet patient needs.

Minimum 1 year of direct care experience is required; NLN-approved program and active RN license are essential. Strong communication and multitasking skills are a must for managing patient care in a home setting.

Qualifications

• Licensed RN with current PA/NJ license (as applicable).
• Minimum one year direct care experience in healthcare.
• Strong assessment and critical thinking abilities.
• Effective communication with patients, families and team.
• Proficiency in documentation and basic computer skills.

Responsibilities

• Performs telephone triage for after-hours calls with clinical judgement.
• Provides direct nursing care and assesses patient status in home care.
• Educates patients and families on end-of-life care, pain control and discharge planning.
• Documents data and orders; communicates with interdisciplinary team.
• Ensures hand-off to next shift and follows up when needed.
