Job summary
Senior Care Solutions is seeking a Per Diem Physical Therapist to join our home health team in the Miami, FL area. You will deliver skilled PT in patients' homes, conducting evaluations, creating plans, and coordinating with an interdisciplinary team to promote recovery and independence.
Responsibilities include OASIS documentation, CMS-compliant assessments, and patient education on exercises, safety, and equipment.
Qualifications
- Current Florida Physical Therapist license in good standing.
- Minimum of one year PT experience; home health experience preferred.
- OASIS documentation experience strongly preferred.
- Current CPR certification.
- Valid driver's license and reliable transportation.
Responsibilities
- Perform comprehensive PT evaluations and ongoing reassessments.
- Complete OASIS SOC/ROC/Recert/Transfer/Discharge assessments per CMS guidelines.
- Develop and implement individualized treatment plans to improve strength, balance, mobility.
- Educate patients and caregivers on home exercise programs, safety and fall prevention.
- Collaborate with physicians and the care team to optimize outcomes.
- Monitor progress, adjust plans as needed and communicate changes.
- Complete clinical documentation accurately in the EMR and ensure regulatory compliance.
