Job summary

Senior Care Solutions is seeking a Per Diem Physical Therapist to provide skilled therapy in patients' homes across Largo, FL and surrounding areas. The PT will perform evaluations, develop treatment plans, and implement evidence-based interventions to improve mobility and function.

The role requires a Florida PT license, at least 1 year of licensed PT experience (home health preferred), Medicare knowledge, and proficiency with EMR systems. Per diem schedule offers flexibility and independence.

Qualifications

Florida PT license is required.
Minimum 1 year licensed PT experience; home health experience preferred.
Medicare documentation requirements should be understood.
Experience with EMR systems; Kinnser/WellSky a plus.
Strong interpersonal, communication, and time management skills.
Ability to work independently on a per diem basis.
Reliable transportation and valid driver's license.

Responsibilities

Conduct in-home physical therapy evaluations and follow physician's plan of care.
Develop and implement individualized therapy plans to restore function and mobility.
Instruct patients and caregivers in safe use of devices and exercises.
Monitor patient progress and modify plans to achieve goals.
Collaborate with nurses, therapists, aides, and physicians for coordinated care.
Maintain timely, accurate documentation in the EMR.
Travel to patient homes across the assigned territory.
