Job summary

UnitedHealth Group is seeking a Per Diem Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for Home Health in Louisiana. The role involves delivering quality patient care in patients' homes, coordinating with the health care team, and educating families.

Required are an active LPN/LVN license in LA and driving with reliable transportation; CPR is preferred or must be completed within 90 days. At least one year of LPN experience is expected.

Qualifications

• Current and unrestricted LPN or LVN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire.
• Home care experience preferred.
• Ability to work flexible hours.
• Proven ability to work independently.
• Proven excellent communication, writing, and organizational skills.
• LA LPN licensure without restrictions per state requirements.
• At least 1 year of LPN experience.

Responsibilities

• Provides services within the scope of practice as defined by state laws and plan of care.
• Instructs patient family in care and home environment maintenance.
• Observes and reports patient responses to treatment to supervisor.
• Maintain ability to work in diverse home environments.
