Job summary

Hackensack Meridian Health is seeking a Registered Nurse for Home Care in Wall, NJ. This per diem role offers flexible scheduling and the chance to impact patients’ lives through individualized home-based nursing.

You will assess patient needs, implement care plans, document thoroughly, and coordinate with physicians to ensure adherence to orders while guiding a care team in a home health setting.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate required.
• RN experience: at least six months or LPN experience for at least one year, preferably in home care or hospice.
• Current NJ State RN license.
• AHA Basic Life Support (BLS) for Healthcare Providers.
• IV certification preferred.

Responsibilities

• Provide professional nursing care to patients in their homes.
• Assess, develop, implement, and evaluate home nursing care needs.
• Ensure compliance with physician orders and documentation.
• Demonstrate holistic care for patients and their families.
• Complete nursing assessments and medication reconciliations.
• Collaborate with physicians on the Plan of Care.
• Serve as a liaison among physicians, management, and healthcare team members.
• Supervise home health aide staff.
• Stay updated on nursing practice standards.
• Complete routine assignments independently and meet productivity requirements.
