Job summary

Hackensack Meridian Health At Home is seeking qualified home health nurses to provide in-home care to patients in New Jersey. The role focuses on delivering individualized nursing care with a team-based approach, ensuring adherence to physician orders and comprehensive documentation.

Required are an RN license in New Jersey or a nursing diploma, with 6 months RN or 1 year LPN experience, preferably in home care or hospice, plus basic PC skills.

Qualifications

• Associate's Degree in Nursing or Diploma Certificate in Nursing.
• Minimum of six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice.
• PC Experience/Skills.

Responsibilities

• Provide professional nursing care to patients in their home through assessment, development, implementation and evaluation of home nursing care needs.
• Assure compliance with physician orders and that visit frequency follows orders; document progress to goals at discharge.
• Document clinical decisions and ensure safe patient care; coordinate with interdisciplinary team.
