Job summary

Hackensack Meridian Health is seeking an RN for Home Care in Wall, NJ. The role provides professional nursing care in patients’ homes, with emphasis on individualized plans, physician-order compliance, and collaboration with interdisciplinary teams.

The ideal candidate holds an Associate’s Degree in Nursing or Diploma in Nursing, has 6+ months of RN experience (or 1 year as an LPN), and possesses NJ RN license plus AHA BLS HCP certification.

Qualifications

• Education: Associate’s Degree in Nursing or Diploma Certificate in Nursing.
• Experience: At least six (6) months RN experience or at least 1 year of LPN experience, preferably in home care or hospice.
• Licenses & Certifications: NJ State Professional Registered Nurse License; AHA Basic Health Care Life Support HCP Certification.

Responsibilities

• Provide professional nursing care to patients in their home.
• Assess, develop, implement, and evaluate home nursing care needs.
• Ensure compliance with physician orders and documentation.
• Demonstrate holistic care for patients and their significant others.
• Maintain safe patient care and demonstrate knowledge in treatments.
• Utilize interdisciplinary teams and document all activities accurately.
• Identify appropriate payors and comply with authorizations.
• Complete nursing assessments and collaborate with physicians on care plans.
• Implement teaching strategies to meet established goals.
• Act as a clinical supervisor for home health aide staff.
