Job summary

Ennoble Care seeks an experienced per diem Hospice Social Worker to service patients in the Dayton, OH region and surrounding areas. You will conduct psychosocial assessments and participate in the patient’s plan of care, ensuring holistic support.

The role requires an LMSW or MSW from a CSWE-accredited program, at least one year of post-master’s social work experience in healthcare, and the ability to handle confidential information.

Qualifications

• Must hold an LMSW license.
• Master’s degree in social work from CSWE-accredited program.
• At least 1 year of post-master’s social work experience in a healthcare setting.
• Hospice experience preferred.
• Ability to reliably handle and maintain confidential information.
• Must have a licensed driver with insured automobile in good working order.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct psychosocial assessment within 5 days of hospice admission.
• Participate in IDG meetings and develop the patient’s plan of care.
• Observe and report changes in emotional and social factors affecting care.
• Maintain and submit written clinical records (initial evaluation, care plan, daily notes).
• Evaluate patient and family response to social work intervention.
• Coordinate scheduling of visits with Case Managers weekly for care coordination.
• Participate in IDG and QAPI meetings and activities.
• Offer support if patient/family declines social work services.
• Engage in self-development through education, workshops, and professional activities.
• Assess coping with dying for patient/family.
• Consult health team on social, emotional, environmental factors.
• Help patients utilize family and community resources.
