Job summary

VNA Health Group in New Jersey is seeking a Staff Nurse Homecare to deliver high‑quality, in‑home care. You will assess patients, develop personalized care plans, and educate families to support healthier lives, all while coordinating with a team of clinicians.

We offer a two‑week orientation, RN with BSN preferred, 1+ year of clinical RN experience (homecare a plus), EMR proficiency, and a valid driver’s license for patient visits.

Qualifications

• Registered Nurse (RN) with a bachelor’s degree in nursing (preferred).
• 1+ year of clinical RN experience; homecare experience is a bonus.
• Strong verbal and written communication skills.
• Proficient with technology and EMR systems.
• Reliable car and valid driver’s license for home visits.

Responsibilities

• Complete two‑week clinical orientation to learn procedures and EMR.
• Assess patients with complex health needs and develop care plans.
• Educate patients and families about conditions and health paths.
• Collaborate with clinicians to ensure seamless home care.
• Document changes in patient conditions and update care plans.
