AltaMed Health Services Corporation in West Covina, CA seeks a part-time Staff Physician, Family Practice (per diem) to deliver high-quality medical care within our Team-Based Care Model. This role focuses on population health, evidence-based practice, and integrated delivery of care.

The ideal candidate will hold a California medical license, board certification or eligibility in Family Practice, and be committed to ongoing professional development, EMR proficiency, and compassionate patient

Qualifications

• Active California medical license.
• Board certification in Family Practice or Board eligible (within 18 months).
• Experience with electronic medical records (EMR).
• Leadership/management experience preferred.
• BLS certification or higher required.

Responsabilités

• Provide medical services to AltaMed patients.
• Deliver care aligned with Team-Based Care Model and population health.
• Utilize evidence-based medicine and integrated delivery of care.
• Monitor resource use and patient outcomes.
