Qualifications

• Current, unrestricted California RN license.
• Minimum 3 years RN experience; home health preferred.
• Proof of negative TB result required.
• Physical screening document within last year.
• Active CA driver's license and auto insurance.
• Phone with internet access required.

Responsibilities

• Holistic patient evaluations addressing physical, mental, and living environment.
• Tailored care roadmaps with milestones based on patient goals.
• Proactive chronic disease oversight to prevent hospital readmissions.
• Skilled nursing interventions including wound care and meds administration.
