Job summary

Constellation Health Services is seeking a compassionate Chaplain to join our Florham Park hospice team. The Chaplain will provide spiritual care to patients and families, coordinating with other spiritual counselors and the interdisciplinary team.

You will assess spiritual needs, participate in care planning, and offer direct care, education, and liaison support. Funeral services and volunteer supervision may be included as part of duties.

Qualifications

• Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) or evidence of completion of three or more units of Clinical Pastoral Education.
• Experience working with death and dying, individuals/family/caregiver.
• Ability to work as member of interdisciplinary group.
• Comfort in an interfaith setting.

Responsibilities

• Perform spiritual assessments for patients and families in the hospice program.
• Contribute to individualized plans of care through comprehensive spiritual assessments.
• Provide direct spiritual care to patients and families/caregivers.
• Serve as liaison to community chaplains and spiritual counselors.
• Maintain records of spiritual care services and activities for QA and program development.
• Attend patient care conferences and interdisciplinary team meetings.
• Provide consultation and education to the interdisciplinary team on spiritual care.
• Arrange funeral or memorial services as requested.
• Supervise spiritual care volunteers when assigned to patients/families.
• Participate in QA and performance improvement activities.

Constellation Health Services is seeking a compassionate and dedicated Chaplain to join our Hospice Agency to provide spiritual guidance to our community. The Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of Constellation Health Services, either directly or through coordination of care with other spiritual counselors.This role would be out of our Florham Park, NJ location.Constellation Health Services was founded to be an innovative leader in the post-acute continuums by the development of core teams that embody clinical excellence, uniqueness within their communities and act with authentic intention.
