Job summary

Qualifications

• Education: Bachelor's Degree Related Field of Study required Can this role accept experience in lieu of a degree? No
• Licenses and Credentials: RN, PT, OT, SLP, RT or LCSW/LICSW required
• Experience: case management, utilization review, or discharge planning experience 2-3 years preferred
• Knowledge, Skills and Abilities: Strong customer and interpersonal skills.
• Knowledge of community service resources.
• Proficient User in Microsoft Office Applications.
• Strong commitment to clinical excellence.
• Strong problem solving skills and team oriented behavior.
• Strong patient/customer service skills.
• Ability to verbally calm patients.

Responsibilities

• Discusses assessment findings with physician to maximize outcomes
• Assist in discharge planning
• Communicating effectively internally and externally
• Coordinates discharge and aftercare with clinical guidance
• Support the team and follow through on requests
• Gather baseline data and identify barriers to post-inpatient care
• Develop realistic post-discharge care plans with patient and care team
• Identify resources for homecare, outpatient, or alternative care
• Evaluate appropriateness of level of care with payer involvement
• Comply with State and Federal regulations and accrediting bodies
