The Center for Urban Community Services (CUCS) is an acclaimed human services organization and one of the nation's largest providers of housing and social services. CUCS is a leader in implementing new practices, sharing knowledge, and assisting in shaping local, state, and national strategies to support persons who are homeless, low-income, living with mental illness, or have other special needs to live successfully in the community. CUCS invites applications for a Per Diem Case Manager at The Hegeman.
The Hegeman is a permanent supportive housing residence located in Brownsville, Brooklyn, for 160 low-income tenants, many of whom have histories of mental illness, homelessness, and substance abuse. The building has a LEED (Leadership in Energy and Environmental Design) silver rating and includes a 24/7 attended lobby, a community garden, a computer room, and a fitness room. CUCS provides comprehensive on-site social services using a person-centered and recovery-oriented approach, which include evidence-based practices such as Supported Employment, Motivational Interviewing, and Wellness Self-Management, with particular emphasis on mental health, chemical dependency, and services to people living with HIV/AIDS. This property is managed by Breaking Ground.
The Case Manager is responsible for working with an assigned group of clients to help them obtain housing, support clients in living full and satisfying lives in the community, assist their team and program in functioning effectively, and meet contractual obligations, including achieving housing placement goals. The role also involves striving, as appropriate, to fulfill the CUCS programs' core purpose, values, and vision statements.
The Per Diem Case Manager works on evening, overnight, or weekend shifts at a program site and does not carry a specific recipient caseload. Instead, this position is responsible for ensuring the comfort and well-being of the entire recipient census of a site by carrying out program activities and providing direct clinical services.
The Per Diem Case Manager is expected to actively support and work toward accomplishing the agency's mission and the program's specific goals and objectives.
Shifts available:
Weekends and Holidays, Fridays 4pm to 7pm, Saturdays 10am to 3pm - bi-weekly
