Job summary

Saint Clare's Health, part of Prime Healthcare, is seeking a Behavioral Health Social Worker to join its award-winning team in New Jersey. This role provides emotional support, crisis intervention and counseling to patients across pediatrics to older adults, coordinating care within an interdisciplinary team.

Responsibilities include psychosocial assessments, treatment planning, discharge planning and thorough clinical documentation.

Qualifications

• Master’s degree in Social Work or related field; licensure required where applicable.
• BLS certification required on hire and maintained.
• Minimum one year inpatient or outpatient behavioral health experience.
• Experience working with diverse populations and complex psychosocial needs.

Responsibilities

• Provide emotional support, crisis intervention and counseling for patients across pediatrics to older adults.
• Perform psychosocial assessments and contribute to treatment planning.
• Coordinate discharge planning and work within an interdisciplinary team.
• Maintain accurate clinical documentation for each client during stay.
