• Evaluate patients and develop rehabilitation plans based on age, diagnosis and chart review.
• Complete documentation timely with progress toward functional goals.
• Plan individual treatment programs following physician orders.
• Provide instruction to team regarding patient functional needs as needed.
• Coordinate with physicians for changes in treatment plan.
