Job summary

Kaweah Health in Visalia, CA seeks a licensed Physical Therapist to evaluate and treat patients across our acute and rehabilitation services.

You will develop individualized rehabilitation plans, perform competency-based assessments, document progress, and collaborate with the care team to maximize patient function and quality of life. This role supports day shift duties in a community-focused hospital setting.

Qualifications

• BLS certification required.
• CA Physical Therapist license required.
• Infant, pediatric, and adolescent unit experience encouraged.
• If assigned to Home Health: valid California driver’s license and auto insurance.

Responsibilities

• Must pass annual competency assessment.
• Administers standard formal evaluations based on age, diagnosis, and chart review.
• Evaluate patient deficits and progress towards functional goals.
• Complete timely documentation of progress and treatment plans.
• Plan and execute individualized treatment programs.
• Perform therapy procedures safely and effectively.
• Educate patients and families on care needs and daily activities.
• Coordinate with physicians for plan changes.
• Ensure equipment is available and properly fitted for patient use.
• Supervise aides and assistants in task completion.
• Participate in continuing education and departmental meetings.
• Meet productivity targets and assist with department orientation.
