Job summary

Terros Health in Phoenix, AZ is seeking a dedicated Peer Support Specialist to join the Mitchell Recovery Health Center. You will engage, educate, and coordinate care with patients, families, and clinicians, supporting recovery goals in a home, clinic, or community setting.

This full-time role involves facilitating peer groups, coordinating transportation, and delivering exceptional service to patients and partners.

Qualifications

• Experience coordinating care and engaging patients in recovery goals.
• Ability to facilitate peer support groups and psychosocial activities.
• Strong communication with patients, families, and providers.

Responsibilities

• Work with the clinical team to engage, educate, and coordinate care with patients, families, and providers.
• Conduct face-to-face interactions in home, community, or clinic settings to assess and support functioning.
• Participate in Consumer Advisory Council and promote patient and family participation.
• Facilitate peer support and psychosocial groups to support individual recovery goals.
• Provide transportation for patients in a personal or Terros Health vehicle when needed.
• Deliver excellent customer service to patients, providers, and other stakeholders.
• Assist patients in finding resources to meet basic needs beyond covered services.
• Encourage patients to express their perspectives during assessment and service delivery.
