Location: Onsite, position may be assigned to work at both IHC locations

SUMMARY

Under the direction and supervision of the SUD Counselor Manager, this position is responsible for assisting patients with referrals toward sobriety/recovery from a substance use disorder (SUD).

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Serve as Peer Support for patients who are in Behavioral Health SUD program. Perform all of the functions of a standard Peer Support Specialist I.
2. Facilitate substance use disorder SUD assessments (ASI), ASAM.
3. Facilitate individual treatment plan (ISP).
4. Facilitate support groups/talking circles.
5. Make referrals to outside agencies.
6. Maintain well-organized documentation, for all contacts in person or telephone.
7. Follow up with patient's missed appointments, coordinate with front desk for scheduling.
8. Complete ROI’s.
9. Collaborate with other IHC department staff, such as health educators and the elder navigator, to plan and implement outreach activities and to ensure integrated care and appropriate outside agencies are identified for the provision of treatment services.
10. Attend department meetings, provide patient program updates.
11. Participate in local community gatherings/events.
12. Participate in Case Management.
13. Assists with various group workshops as planned by the SUD Counselor Manager.
14. Facilitate the following core functions, 1) Patient screening, 2) Patient intake, 3) Patient orientation, 4) Patient assessment, 5) Treatment planning, 6) Case management, 7) Referrals, 8) Record keeping and reporting, 9) Cross- consultation.

SUPERVISORY RESPONSIBILITIES

This position has no supervisory responsibilities.

TRAVEL & DRIVING REQUIREMENTS

• Travel may be required for this position.
• Driving is required for this position.

EDUCATION AND/OR EXPERIENCE

• High school diploma or GED.
• Peer Support Specialist Certification required.
• RADT (CCAPP).

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

• Knowledge of:
  • Social, emotional, and behavioral aspects of behavioral health disorders and illness.
  • Behavioral health care systems and services.
  • Principles and practices of building and maintaining strong relationships and connections with peers.
  • Principles and practices of direct client service delivery.
  • Principles of behavioral health education. Principles and practices of case management for behavioral health clients, including client charting and process documentation. Cultural, social, and environmental factors and influences affecting behavioral health. Community and governmental services and resources.
  • Basic needs and challenges for individuals and families accessing behavioral health services, including cultural barriers.

CERTIFICATES, LICENSES, MEMBERSHIPS AND REGULATIONS

• Peer Support Specialist certification.
• Active membership in a consumer advocacy or self-help group.
• A valid California driver’s license and a good driving record will be required at the time of appointment and must be maintained throughout employment.
• Current AED CPR certification or attainment within 6 months of hire.
• Applicants must successfully pass a pre-employment physical exam, tuberculin skin test or x-ray and urine drug screen test that includes marijuana.
• Health must be adequate to perform all duties of the position.
• Must pass a criminal background check.
