Job summary

Freedom House Recovery Center, Inc. in Lake Area, NC, seeks a dedicated behavioral health professional to provide interventions that promote safety, self-worth, and growth.

This full-time/part-time role focuses on assisting consumers transitioning from crisis or inpatient settings to outpatient treatment. You will collaborate with clinicians and discharge planners to develop and implement treatment plans, engage clients in skill-building and wellness activities, and ensure timely documentation

Qualifications

• Provide interventions to promote safety, self-worth, and growth.
• Assist consumers with transition from crisis or medical settings to outpatient treatment.
• Develop and implement treatment plans with clinicians or discharge planners.
• Engage consumers in activities for skill development and wellness education.
• Initiate and maintain contact to facilitate service transitions.
• Provide transportation to initial appointments when needed.
• Maintain accurate documentation and assist with service authorizations.
• Maintain confidentiality and comply with service definitions.

Responsibilities

• Provide or assist with transportation to initial appointments.
• Maintain and submit accurate and timely documentation requirements and assist with service authorizations as needed.
• Maintain confidentiality and comply with service definition.
• Other duties and responsibilities as assigned by immediate supervisor.
