Community Bridges, Inc. (CBI) in Portland, OR is seeking a Continuing Care Peer with lived recovery experience to provide community-based outreach, support, and care coordination for individuals facing behavioral health challenges and homelessness.
This role supports deflection and sobering services, helps with transitions to appropriate care, and ensures follow-up and advocacy while promoting dignity and continuity of services within a compassionate team.

The Continuing Care Peer is an individual with lived experience in recovery and who provides community-based outreach, support, navigation, and care coordination for individuals experiencing behavioral health challenges, substance use, homelessness, or crisis situations. This position provides peer support, outreach, care planning, and resource linkage both onsite and in the community. This Continuing Care Peer supports individuals during deflection encounters, sobering services, and transitions to appropriate levels of care, while ensuring follow‑up, advocacy, and continuity of services.
