Scheduled Hours: 8a-4p

Shift: 1 - Day Shift, 7.5 Hours (United States of America)

CSP Case Managers meet with referred clients in various community settings, complete intake assessments, develop person-centered service plans, and assist clients in achieving their goals. Responsibilities include researching community resources, providing support such as transportation, advocacy, skills building, coaching, and completing paperwork.

Responsibilities
  1. Work effectively as part of a team to support Persons in articulating and developing their recovery goals.
  2. Provide services that meet agency requirements and client needs.
  3. Document service activities according to agency standards.
  4. Partner with other team members to support recovery processes.
  5. Assist and empower Persons using mental health and addiction services.
  6. Facilitate mutual support and self-help groups.
  7. Guide Persons in developing WRAP or crisis plans.
  8. Support Persons in preparing for meetings and social events.
  9. Build relationships with community agencies to support recovery.
  10. Participate in workshops and training to stay current with recovery practices.
  11. Perform other related duties.
  12. Attend staff meetings and daily rounds.
  13. Provide transportation to clients as needed, including using personal or program vehicles.
  14. Respect diverse views, demonstrate professionalism, tolerance, civility, and acceptance.
Qualifications
  • Grade 5: Case Manager I: High School Diploma/GED/HiSet; Associates degree preferred.
  • Grade 6: Case Manager II: Bachelor’s degree, preferably in psychology or human services.
  • Previous/current participation in the MH system as a service recipient and willingness to convey this experience. Must be certified or eligible for the Mass Peer Specialist training and certified within 6 months.
  • Lived Experience Required.
  • Bilingual candidates are strongly encouraged to apply.
  • Valid US driver’s license and ability to provide insured, inspected vehicle for work-related transportation.
  • Ability to pass a CORI background check.
