  • Complete client engagement, needs assessments, service planning, follow-up, and coordination of care in collaboration with clients and the multidisciplinary team.
  • Assist clients with accessing and maintaining public benefits, entitlements, health insurance, food resources, transportation, housing-related supports, vocational services, and other community resources.
  • Provide referrals and linkages to behavioral health, substance use disorder, medical, social service, and community-based programs as appropriate.
  • Advocate with and educate clients on how to advocate for themselves as they navigate healthcare providers, government agencies, benefits systems, and other service systems.

  • Provide peer support grounded in mutuality, respect, shared decision-making, self-determination, and individual choice.
  • Help clients identify strengths, recovery goals, coping strategies, wellness tools, triggers, and natural supports.
  • Provide active listening and recovery-oriented support to clients experiencing anxiety, emotional distress, isolation, stigma, or other behavioral health challenges.
  • Support clients in strengthening self-advocacy, problem-solving, organizational, communication, and independent living skills.
  • Facilitate or co-facilitate peer support, recovery, wellness, and psychoeducational groups as assigned.
  • Recognize changes in client presentation that may indicate increased behavioral health needs or safety concerns; provide supportive engagement and de-escalation within the scope of the role and follow CRW procedures for urgent concerns.
  • Collaborate with clinicians, medical providers, care coordinators, and other members of the client’s support network when appropriate and with proper consent.
  • Complete required case notes, service documentation, assessments, and other records accurately and within established program, agency, and regulatory timeframes.
  • Participate in supervision, case conferences, interdisciplinary meetings, trainings, and program meetings as required.
  • Maintain client confidentiality, professional boundaries, and the distinct values and principles of peer support while functioning as an integrated member of the CRW team.
