The OMNIA Peer Specialist is part of an initiative to assist homeless individuals with substance use disorder gain access to treatment, benefits, and housing. The Peer Specialist will conduct outreach, engagement, and screening to identify and enroll eligible clients, using evidence-based approaches to assess and assist clients on their caseload to address substance use disorder and chronic homelessness.

Locations: Wards Island, NY 10035; NY, NY 10020

Minimum Qualifications
  • High School diploma or GED.
  • Personal experience with and knowledge of the challenges related to substance use and/or mental health conditions. Certified Recovery Peer Advocate (CRPA) certification preferred.
  • At least 2 years of work experience and familiarity with the service systems used by the target population.
  • Knowledge of medication-assisted treatment, opiate overdose prevention, and substance use disorder treatment and recovery services.
  • Excellent engagement skills, boundary setting, and follow-up abilities. Strong verbal and written communication skills, including basic computer skills.
  • Fluency in Spanish or another second language is a plus.
  • Ability to engage with chronic homeless individuals at various locations such as welcome centers, shelters, treatment facilities, hospitals, and other sites identified by the project team and stakeholders.
  • Experience in completing screenings, assessments, and service planning using evidence-based methods.
  • Assist clients with completing necessary documentation, applications, and tasks related to their goals.
  • Support clients by accompanying them to appointments for housing, health, behavioral health, and income services, providing advocacy, coaching, and support.
  • Deliver services in accordance with program protocols and evidence-based models.
  • Re-engage clients who have been lost to follow-up through outreach efforts.
  • Maintain accurate documentation including case notes, release forms, client tracking, assessments, and related documents.
  • Co-facilitate outreach and engagement groups, presenting on topics related to substance use treatment, mental health, and harm reduction.
  • Participate in case conferences, scheduled appointments, and follow-up activities.
