Qualifications

• Help clients identify and access community resources upon discharge.
• Support navigation of the care continuum and advocate for client needs.
• Develop personal recovery goals and action plans with clients.
• Collaborate to identify strengths, overcome barriers, and celebrate progress.
• Maintain accurate records of interactions, progress notes, and activities.
• Ensure documentation meets organizational and regulatory standards and confidentiality.
• Submit regular reports on progress and outcomes to supervisors.
• Perform miscellaneous duties as directed.

Responsibilities

• Provide one-on-one peer support to clients, drawing from personal experience to build trust and rapport.
• Staff available community spaces/living rooms to increase engagement to address barriers to care.
• Share insights on navigating recovery, coping with challenges, and building resilience.
• Serve as a positive role model, demonstrating healthy coping skills and self-advocacy.
• Help clients identify and access community resources upon discharge.
• Support clients in navigating the continuum of care and advocating for their needs during treatment.
• Assist clients in developing personal recovery goals and action plans to support their wellness journey.
• Collaborate with clients to identify strengths, overcome barriers, and celebrate progress.
• Maintain accurate and timely records of client interactions, progress notes, and activities.
• Ensure documentation meets organizational and regulatory standards, while respecting client confidentiality.
• Submit regular reports on client progress, engagement, and outcomes to supervisors and program stakeholders.
• Miscellaneous Duties as directed.

Education and/or Experience

High school diploma or equivalent; certification as a Peer Specialist (CPS). Lived experience with mental health and/or substance use recovery and a demonstrated ability to manage personal wellness.
