Responsibilities

• Provide outreach and engagement with individuals with substance use disorders.
• Support parents/caregivers to achieve recovery goals and develop a Recovery Plan.
• Link clients to community supports and transportation; assist with daily living skills as needed.
• Collaborate with family team meetings and case workers to ensure coordinated services.
• Maintain confidentiality and document activities in required systems.
