Responsibilities

• Model recovery-focused relationships within professional boundaries.
• Assist orientation and bridge to enrollment for SUD services.
• Provide case management, outreach and life-skills coaching.
• Develop and implement individual treatment plans with providers.
• Deliver one-to-one and group recovery training and outreach.
• Arrange or provide transportation to services and appointments.
• Keep the treatment team informed about client progress and obstacles.
• Expand knowledge of community resources and advocate for clients.
• Screen and intervene for trauma; refer in crisis situations.
