Job summary

Responsibilities

• Provides recovery education to patients for every phase of the recovery journey.
• Works with local law enforcement and emergency service professionals for home visits.
• Maintains follow-up contact to support ongoing recovery and re-engagement.
• Transports patients to MAT appointments when necessary.
• Assists patients in articulating goals, steps and barriers for recovery.
• Maintains patient confidentiality and professional boundaries.
• Fulfills documentation requirements accurately and timely.
• Travels daily to meet with individuals within the assigned county; on-call schedule.
