Job summary

Baltic Street Wellness Solutions seeks a Peer Support Specialist to assist adults moving from adult homes into the community across NY's five boroughs. You will leverage lived experience to foster recovery and self-management.

Required is at least one year as a peer and NYS certification or progress toward it, plus strong communication skills and ability to travel to multiple homes, including evenings and weekends.

Qualifications

• High School Diploma or GED required; college degree preferred.
• At least one year of experience as a peer.
• NY State Certified Peer Specialist or certification within 6 months.
• Knowledge of NYS mental health and substance abuse systems.
• Ability to be a role model and convey hope and wellness.
• Ability to complete basic documentation requirements.
• Must have lived experience with the public mental health system.
• Basic computer skills; Foothold AWARDS knowledge a plus.

Responsibilities

• Offer supportive peer relationships that foster trust and hope for change.
• Provide one-on-one and/or group peer support after relocation.
• Teach community adjustment and wellness self-management skills.
• Assist with developing a Wellness Recovery Action Plan (WRAP).
• Help identify and re-engage with family and natural supports.
• Provide wellness education and self-advocacy resources.
• Continue with individuals after transition to access community supports.
• Complete detailed written records of activities in a timely manner.
• Report to and meet with supervisor regularly.
• Attend required trainings and meetings.
