Responsibilities

The Licensed Practical Nurse in Home and Community-Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by the physician or authorized by the state/program, complying with applicable laws, regulations, and policies. The RN coordinates the care plan with other healthcare professionals and ensures continuity of care through communication with physicians, agency staff, patients, families, case managers, and community resources.

Education and Experience

Provides high-quality clinical services within the scope of practice and infection control standards, following the care plan, and coordinating with the care team from admission to discharge.

Completes clinical nursing assessments as required by federal and state programs and payers.

Ensures patient eligibility and medical necessity for services per payer and agency policies.

Develops and revises individualized care and service plans according to federal and state requirements with community providers.

Ensures the care plan's frequency and duration meet patient needs and revises plans with physician approval as necessary.

Experience Desired

A minimum of one year of RN experience; pediatrics experience preferred.

License Requirements

Current RN licensure in the state of practice.

Valid driver’s license, vehicle insurance, access to a dependable vehicle or public transportation.

Current CPR certification.
