Responsibilities

The Licensed Practical Nurse (LPN) in Home and Community Based Services provides and directs nursing and personal care to patients in their homes, as prescribed by physicians and in compliance with all applicable laws, regulations, and agency policies. The RN coordinates the care plan with other healthcare professionals and ensures continuity of care through effective communication with physicians, agency staff, patients, families, case managers, and community resources.

Education and Experience

Deliver high-quality clinical services within your scope of practice, adhering to infection control standards, and in coordination with the care team from admission to discharge. Complete clinical assessments as required by federal and state regulations and payers. Ensure patient eligibility and medical necessity for services. Develop and update individualized care plans in compliance with regulations, adjusting as needed with physician approval.

Experience Desired

Minimum one year of RN experience; PEDS experience preferred.

License Requirements

Current RN licensure in the state of practice. Valid driver’s license, vehicle insurance, access to reliable transportation, and current CPR certification.
