Qualifications

• LMSW license (Texas State Board of Social Work Examiners); temporary license acceptable during first 90 days; CPI training to be completed within 30 days of hire.
• Preferred: 2 years related social work experience.

Responsibilities

• Participates in multidisciplinary rounds and determines patient needs for discharge planning, including transportation and post-acute care.
• Uses crisis intervention and problem-solving to assist patients and families in meeting medical and psychosocial needs.
• Maintains documentation and referrals per department standards.
• Identifies and reports situations to governmental agencies as required.
• Collaborates with health care team to support patient care and transition planning.
• Provides education to staff, patients, and community resources.
• Pursues professional growth opportunities and participates in Performance Improvement activities.
• Performs other duties as assigned.
