Angels of Care Pediatric Home Health in Tampa, FL is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) for our Pediatric Transitional Nurse Team to support medically fragile children during critical transitions to home care.

This role requires high-acuity pediatric care, with expertise in Trach, Ventilator management, G-tube, and emergency management. Positions offer day and night shifts with biweekly pay and local travel as needed.

Qualifications

• Active, unrestricted RN or LVN/LPN license (FL) is required.
• Current CPR certification.
• Ability to lift and assist pediatric patients as needed.
• Pediatric home health experience is preferred.
• Experience with Trach care, Ventilator management, and TPN.
• Care for high-acuity pediatric patients.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients' homes.
• Perform patient assessments and ongoing evaluations; administer medications, treatments, and therapies.
• Support hospital-to-home transitions for high-acuity patients.

This role is ideal for nurses who are confident working withhigh-acuity patients andskilled in trach, vent, G-tube, and emergency management.

As a Transition Nurse, you'll play a critical role ensuring safe, successful discharges, preventing readmissions, providing interim patient coverage, and supporting continuity of care until permanent staffing is established.

What You’ll Do

• Provide skilled pediatric nursing 1:1 care in patients’ homes

Required:

• Active, unrestricted Registered Nurse (RN) or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license (FL)
• Current CPR certification
• Ability to lift and assist pediatric patients as needed
• Pediatric home health experience
• Experience with Trach care, Ventilator management, and TPN
• High-acuity pediatric patients

Schedule & Travel

• Full-time position, with guaranteed hours
• Day and Night shifts available
• On call and Staffing coverage responsibilities as needed
• Field-based role with local travel
