Job summary

Angels of Care Pediatric Home Health in Memphis, TN is seeking a Pediatric Transitional Nurse to support medically fragile children during transitions to home care. RN and LVN/LPN roles available, with day and night shifts, on-call responsibilities, and field-based travel within local area.

Ideal candidates are confident with high-acuity patients, trach, vent, G‑tube, and emergency management, and will help ensure safe discharges and continuity of care until permanent staffing is established.

Qualifications

• Active, unrestricted Registered Nurse or LVN/LPN license (TN)
• Current CPR certification
• Experience with Trach care, Ventilator management, and TPN
• Emergency response management

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes
• Perform patient assessments and ongoing evaluations
• Administer medications, treatments, and therapies
• Support hospital-to-home transitions for high-acuity patients
• Educate and support parents, families, and caregivers
• Assist with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Provide first-shift and interim coverage as needed
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately in emergency situations and changes in patient condition

• RN $37-$40/hr

As a Transition Nurse, you'll play a critical role ensuring safe, successful discharges, preventing readmissions, providing interim patient coverage, and supporting continuity of care until permanent staffing is established.

• Ability to lift and assist pediatric patients as needed
• Experience with Trach care, Ventilator management, and TPN
• Emergency response management

Preferred:

• Pediatric home health experience
• High-acuity pediatric patients (preferred)

Schedule & Travel

• Full-time position, with guaranteed hours
• Day and Night shifts available
• On call and Staffing coverage responsibilities as needed
• Field-based role with local travel
