Job summary

Angels of Care Pediatric Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) for our Pediatric Transitional Nurse team in the Philadelphia area. You will provide 1:1 skilled pediatric nursing in patients’ homes, focusing on safe discharges, post-discharge coverage, and continuity of care for medically fragile children.

The ideal candidate has pediatric home health experience, CPR certification, and comfort with high-acuity patients, trach care, and ventilator

Qualifications

• Active, unrestricted RN or LVN/LPN license (PA).
• Current CPR certification.
• Experience with Trach care and Ventilator management.
• Pediatric home health experience.
• Care of high-acuity pediatric patients.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes.
• Perform patient assessments and ongoing evaluations.
• Administer medications, treatments, and therapies.
• Support hospital-to-home transitions for high-acuity patients.
• Educate and support parents, families, and caregivers.
• Assist with discharge support and care coordination.
• Train and support permanent field nurses during transitions.
• Provide first-shift and interim coverage as needed.
• Collaborate with Clinical Care Managers and interdisciplinary teams.
• Respond appropriately in emergencies and changes in patient condition.

Angels of Care Pediatric Home Healthis hiring Registered Nurses (RN) and Licensed Practical Nurses (LPN) for our Pediatric Transitional Nurse Teamto support medically fragile children and their families during critical transitions to home care.

This role is ideal for nurses who are confident working withhigh-acuity patients andskilled in trach, vent, G-tube, and emergency management.

As a Transition Nurse, you'll play a critical role ensuring safe, successful discharges, preventing readmissions, providing interim patient coverage, and supporting continuity of care until permanent staffing is established.

Qualifications

Required:

• Active, unrestricted Registered Nurse (RN) or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license (PA)
• Current CPR certification
• Ability to lift and assist pediatric patients as needed
• Pediatric home health experience
• Experience with Trach care and Ventilator management
• High‑acuity pediatric patients

Preferred:

• TPN experience

Schedule & Travel

• Full‑time position, with guaranteed hours
• Day and Night shifts available
• On call and Staffing coverage responsibilities as needed
• Field‑based role with local travel
