Angels of Care Pediatric Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) to join our Pediatric Transitional Nurse Team in Tampa, FL. You will provide 1:1 pediatric home health care and help families transition from hospital to home.
Ideal candidates have high-acuity pediatric experience, can manage trach, vent, G-tube, and emergency situations, and are committed to safe discharges and continuity of care.

As a Transition Nurse, you'll play a critical role ensuring safe, successful discharges, preventing readmissions, providing interim patient coverage, and supporting continuity of care until permanent staffing is established.
