Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Practical Nurses (LPN) for our Pediatric Transitional Nurse Team. You will provide 1:1 pediatric home care, working with high-acuity patients and families during critical transitions to home care, with day and night shifts available in Jacksonville, FL.

Ideal candidates have pediatric home health experience, CPR certification, and the ability to manage trach/vent care, with field-based travel and guaranteed hours.

Qualifications

• Active, unrestricted RN or LVN/LPN license (FL)
• Current CPR certification
• Ability to lift and assist pediatric patients
• Pediatric home health experience
• Trach care, Ventilator management, and TPN experience
• High-acuity pediatric patients

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients' homes
• Administer medications, treatments, and therapies
• Support hospital-to-home transitions for high-acuity patients
• Educate families and caregivers
• Assist with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Provide first-shift and interim coverage as needed
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond to emergencies and changes in patient condition

• RN - $37~$40

Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Practical Nurses (LPN) for our Pediatric Transitional Nurse Teamto support medically fragile children and their families during critical transitions to home care.

This role is ideal for nurses who are confident working withhigh-acuity patients andskilled in trach, vent, G-tube, and emergency management.

As a Transition Nurse, you'll play a critical role ensuring safe, successful discharges, preventing readmissions, providing interim patient coverage, and supporting continuity of care until permanent staffing is established.

What You’ll Do

Required:

• Active, unrestricted Registered Nurse (RN) or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license (FL)
• Current CPR certification
• Ability to lift and assist pediatric patients as needed
• Pediatric home health experience
• Experience with Trach care, Ventilator management, and TPN
• High-acuity pediatric patients

Schedule & Travel

• Full-time position, with guaranteed hours
• Day and Night shifts available
• On call and Staffing coverage responsibilities as needed
• Field-based role with local travel
