Job summary

AHMC HealthCare seeks a licensed Vocational Nurse (LVN) to support nursing assessments, care planning, and direct patient care in a collaborative hospital setting in California.

You will document data, assist in plan development, and implement care with the RN and health care team while communicating effectively with patients and families. Requires California LVN license, BLS, and PALS; 1+ year of nursing experience, with acute care preferred.

Qualifications

• One year skilled nursing experience required.
• One year acute care experience preferred.
• Current California LVN license, BLS and PALS required.

Responsibilities

• Assesses and documents patient data.
• Assists RN in development of an individualized care plan.
• Implements delegated portions of the care plan and coordinates with the health care team.
• Evaluates patient response to the plan of care and adjusts as needed.
• Maintains accurate documentation and communicates changes to team.
• Participates in patient and family teaching and care coordination.
