Job summary

Qualifications

• Current California LVN license and BLS/PALS certification required.
• Minimum of one year skilled nursing experience; one year acute care preferred.

Responsibilities

• Assist RN with nursing assessment and document patient data.
• Develop collaborative, individualized patient care plans.
• Provide nursing care with clear priority and implement plan.
• Record interventions clearly in medical records.
• Explain tests, procedures, and medications to patients and families.
• Evaluate patient response and revise care plans as needed.
• Participate in reports, conferences, and interdepartmental relations.
• Uphold safety, infection control, and professional standards.
