Job summary

CCMC Cook Children\'s Medical Center is seeking a Solid Organ Transplant Clinical Pharmacy Specialist to support pediatric renal and cardiac transplant patients in both ambulatory and inpatient settings. The role requires deep expertise in transplant and renal pharmacotherapy and the ability to develop individualized treatment plans.

Schedule: Mon-Fri 8:00am-4:30pm; serves as primary contact outside normal hours. Sign-on bonus and relocation assistance provided.

Qualifications

• PharmD from an ACPE accredited school is required.
• Completion of PGY1 and PGY2 in Solid Organ Transplant or Pediatric Pharmacy preferred, or 5 years of equivalent clinical experience.

Responsibilities

• Develop and implement comprehensive medication regimens for renal and/or cardiac transplant patients.
• Monitor therapy responses and adjust regimens, including therapeutic drug monitoring for immunosuppressants.
• Provide specialized counseling and education to transplant recipients, pediatric patients, and caregivers.
• Collaborate with nephrology, cardiology, and transplant teams to optimize outcomes.
• Participate in pre- and post-transplant evaluations and care for heart and kidney recipients.
• Manage adverse events, infection prophylaxis, and drug interactions.
• Provide consultative services on transplant and complex renal pharmacotherapy to staff.
• Stay current with guidelines; contribute to quality improvement and education programs.
