Job summary

Lumiere Children's Therapy Chicago is seeking an extraordinary Licensed Clinical Social Worker to join our interdisciplinary team in Chicago, IL. The social worker will coordinate and provide social services for patients of all ages within our pediatric setting, collaborating with physicians and other clinicians to support holistic care.

Qualifications

• Master’s degree in social work is required.
• LCSW license in Illinois is required.
• At least one year of social work experience in a healthcare setting is preferred.
• Must have reliable transportation for home visits.

Responsibilities

• Develops and maintains the social service program and policies.
• Maintains knowledge of referral resources and coordinates with physicians.
• Conducts home visits to assess psychosocial status.
• Documents progress notes and maintains records.
• Collaborates with Lumiere team on treatment plans and discharge plans.
• Represents the agency with community resource groups and committees.
