Job summary

Navian Hawaii seeks a Pediatric Social Worker II to provide psychosocial support, care coordination, and resource navigation for children with serious illness and their families enrolled in the Pediatric Palliative and Hospice Care Program.

You will work closely with the pediatric interdisciplinary team to develop family-centered care plans, guide families through diagnoses and treatment options, and connect them with community resources and financial assistance.

Qualifications

• Current Hawai'i-licensed LCSW required.
• Minimum 3 years of direct patient care experience.
• Experience with pediatric, hospice, or palliative care populations preferred.
• Background check and screenings required.

Responsibilities

• Conduct comprehensive psychosocial assessments of pediatric patients and families.
• Provide counseling, crisis intervention, and anticipatory grief support.
• Collaborate with pediatric team to develop family-centered care plans.
• Assist families in understanding diagnoses, treatment, and resources.
• Support discussions on illness, loss, and end-of-life with children and siblings.
• Facilitate care coordination among providers, schools, and community agencies.
