Job summary

The Nemours Foundation in Orlando, FL is seeking a full-time Social Worker (MSW) to support the emergency department and inpatient hospital needs. The role focuses on psychosocial care for patients and families navigating kidney disease and dialysis treatment.

You will conduct biopsychosocial assessments, provide crisis intervention, coordinate services, and collaborate with the care team. A CSWE-accredited MSW and at least one year of social work experience are required; 3+ years preferred.

Responsibilities

  • Provide comprehensive psychosocial services to patients admitted to Nemours Children’s Hospital; focus on psychosocial needs and well-being.

  • Offer crisis intervention services, including domestic violence and psychiatric emergencies.
  • Coordinate care with interdisciplinary teams and assist with discharge planning to avoid delays.
  • Support families in navigating healthcare systems and linking to community resources.

Job description

Nemours is seeking a Social Worker (MSW) Full-Time to join our team in Orlando, Florida.

The Social Worker will support the emergency department and inpatient hospital needs.

The Social Worker will serve as a key member of the dialysis care team, providing psychosocial support and care coordination to help patients and families successfully navigate the lifelong challenges associated with kidney disease and dialysis treatment.

Job Duties:
  • Provide comprehensive psychosocial services to patients admitted to Nemours Children’s Hospital. The primary focus will be addressing patients’ psychosocial needs and enhancing their overall well-being. Additionally, play a crucial role in facilitation patient and family adjustment to illness, ensuring optimal functioning for both individuals and families.
    • Offer compassionate and evidence-based psychosocial support to children with various diagnoses, including medical illness, ADHD, depression, anxiety and autism spectrum disorders.
    • Assist families in navigating the healthcare system effectively.
    • Link families with a range of services tailored to meet their unique needs.
    • Apply age specific principles of growth and development to your practice.
    • Collaborate seamlessly with all members of the healthcare team, fostering effective communication and teamwork.
  • Provide psychosocial assessments, intervention, coordination, and collaboration with interdisciplinary teams.
    • Conduct comprehensive assessments of patients and their families’ biopsychosocial situations.
    • Establish clear goals and periodically re-assess the patient/family situation.
    • Document thorough psychosocial assessments and histories for patients and families.
    • Provide timely crisis intervention services, including cases involving domestic violence, psychiatric emergencies, child abuse, and family disputes.
    • Assess patient mental health needs and refer for counseling and psychiatry as needed (both internally and to external community providers).
    • Perform admission screenings and psychosocial assessments.
    • Offer case management, medical crisis counseling, patient/family education, advocacy, residential placement, community referral facilitation, crisis intervention, and mental health evaluation.
    • Assist with the assessment and coordination of child abuse investigations within the hospital.
    • In collaboration with the medical team, manage domestic violence situations.
    • Access social barriers to discharge, work collaboratively with case management, participate in rounds, and elevate complex cases to avoid discharge delays.
    • If on-call, promptly respond to events and service requests within 5 minutes of receiving a phone call.
    • Remain within a 30-minute drive of the hospital.
  • Other duties as assigned.
Job Requirements
  • Masters Degree in Social Work from a program accredited by the Council on Social Work Education (CSWE) is required.
  • Minimum one (1) year of social work experience required; 3+ years is preferred. (In lieu of one year of experience, graduate school internship may be applied).
Schedule

Sunday - Wednesday 2:00PM - 12:00PM
