Qualifications

• Master's degree in Social Work required.
• Licensure as LCSW or LICSW required.
• Clinical experience in a medical setting preferred but not required.

Responsibilities

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
• Forms biopsychosocial assessment, disposition, and treatment plans.
• Collaborates with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitors, evaluates, and records client progress according to measurable goals in the treatment plan.
• Provides short-term, evidence-based psychotherapy (individual, groups) to pediatric patients in the practice in-person and virtually.
• Screens patients and develops comprehensive assessments and treatment plans for patients engaged in behavioral health treatment.
• Delivers a range of interventions to support primary care mental health and behavioral needs including crisis intervention, care coordination, referrals, and safety planning.
• Assesses patients presenting with new mental health symptoms to identify best treatment options.
• Participates in formal/informal case reviews and interdisciplinary meetings.
• Works closely with pediatric primary care staff to provide timely, appropriate care.
• Advocates for school-related needs as applicable to behavioral health diagnoses.
