Qualifications

• Graduate of an accredited School of Nursing
• Active, unrestricted RN licensure in CT
• Intermediate level of computer proficiency in Microsoft Office

Responsibilities

• Provide and document patient care such as injections and preventive screenings
• Triage patients according to approved protocols and scheduling guidelines
• Prepare patients by collecting and recording clinical information
• Educate patients about illness or injury and self-management
• Carry out standing orders consistent with policy
• Document all patient interactions accurately in the EHR
• Coordinate outreach to patients for appointments and test results
• Answer and document patient calls and route to providers as needed
