Job summary

Children’s Healthcare of Atlanta is seeking an experienced Registered Nurse for a patient-care coordination role within the GI clinic focusing on IBD patients. The position emphasizes discharge planning, home healthcare coordination, and collaboration with clinical teams to ensure safe, high-quality care.

Ideal candidates will have BSN or RN with 5 years pediatric experience, current Georgia RN licensure (multi-state eligible), and case management certification within stated timelines.

Qualifications

• Bachelor of Science in Nursing required.
• GA RN licensure or multi-state via compact required.
• Case management certification within specified timeframes.
• BLS certification within 30 days of employment.
• 3 years of pediatric RN experience; BSN preferred or BSN within 2.5 years for current Georgia RN with an Associate’s degree.

Responsibilities

• Completes initial screen of all patients within 24 hours of admission using trigger criteria.
• Develops and implements a robust transition care plan with the clinical team.
• Coords communication among medical staff, social workers, and home health agencies to drive care coordination.
• Facilitates care conferences, interdisciplinary rounds, and other meetings.
• Performs outpatient and clinic care coordination as patients transition between settings.
